Self-Managed Care - WRHA Home Care Program

Manitoba is the sole province that offers the Self and Family Managed Care Program (SFMC) to qualified home care recipients. This initiative allows you to obtain funding to hire private caregivers or a home care company such as Winnipeg Home Care. This gives you the power to choose who enters your home, ensuring consistent, high-quality care. 

 

How Self Managed Care Works:

  1. Eligibility: ​​​

    • Require service to remain safely in your home

    • Require more assistance than is available from existing support /resources.

  2. Require health services or assistance with daily living

  3. Be registered with Manitoba Health

  4. You must live in your own home, apartment, or assisted living facility.

  5. Application:

    • Speak to your existing home care Case Coordinator about the SFMC program or call the Intake Line directly at (204) 788-8330 to apply.

    • Complete the application form. You can ask Winnipeg Home Care for assistance with the application.

  6. Assessment: Your coordinator will determine the number of care hours you are eligible for.

  7. Manage Funds: Open a separate bank account to manage the funds.

  8. Funding: Funds will be provided and are subject to regular audits (we will send you all necessary invoices).

  9. Support: Your RHA will continue to provide necessary equipment and supplies (e.g., gloves, pads) and visits from Nurses and Community Therapy Services.

By hiring a home care company, we handle all the complex tasks, such as hiring staff, managing payroll deductions and submitting to CRA, managing employment contracts, filing and submitting WCB, and replacing staff during holidays.

How much funding is typically provided?

The amount of funding varies based on the assessed care needs of each individual. On average, it ranges from $1,500 to $4,000 per month. This funding is used to cover the costs of hiring private caregivers or a home care agency.

What types of care is typically approved?

  • Home Care Assistants (HCAs): They assist with daily living activities such as bathing, dressing, and meal preparation.

  • Home Support Workers: They provide support with household tasks, companionship, and supervision.

  • Nurses: They offer medical care, including medication administration, wound care, and health monitoring.

How long does the application process take?

It can take between 3-10  weeks until funding is received. The estimated timelines are as follows:

  • Speaking to your case coordinator or calling the WRHA intake line: 1-2 weeks

  • Completing the application form: 1-2 weeks

  • Assessment by Coordinator: 2-4 weeks

  • Opening a separate bank account: 1-2 weeks

What if I have questions or need assistance during the process?

If you have any questions or need help at any stage of the process, Winnipeg Home Care is available to assist you. They can help with the application, managing funds, and ensuring all necessary paperwork and requirements are met.

Does the person I’m applying for have to be my parent?

No, the person in charge of family managed care could be a trusted friend. The person applying as Manager must be approved by the corresponding Manitoba Regional Heath Authority.

Can a manager hire family members?

Manitoba Health and Healthy Living policy states that hiring family members is not permitted. There are some exceptions in unique circumstances and the Director of Home Care (WRHA) must approve it.
